What is the Concern Form?

Our Concern Form is designed for anyone who engages with NSCD to provide us with feedback.

This form has been designed to provide a space for individuals to inform NSCD of any concern such as harassment, sexual misconduct or micro-aggressions they may experience, or any other feedback they wish to communicate.

Who can complete the form?

We invite anyone from the following groups to complete this form:

• Staff, including those on ongoing contracts, employed through agencies, self-employed and guest artists, volunteers and prospective staff
• Students, including those in higher education, pre-vocational training, taking part in learning and participation activities, prospective students and alumni
• Partners, including organisations, dance companies, artists and contractors
• Audience, including any member of the public viewing any performances connected to us, either on- or off-site
• Communities, including members of the public and those belonging to artistic/dance communities

A report can be made with your name or anonymously.  If you report with your name and contact details we will be in touch.

What next?

The content we receive from these forms helps us to address concerns, support individuals and understand any unconscious or implicit bias taking place. It informs necessary action and allows us to better understand issues within our community. All concerns will be logged and appropriately actioned, and this information will be reported and monitored by NSCD Committees.

Any information you provide will remain private and confidential, unless we believe there is a serious safety or welfare concern.
